The analysis identifies four key mechanisms: state legislation, Medicaid contracts, 1115 waiver programs, and state guidance to improve health outcomes and advance equity. It underscores the importance of collaboration between health plans, providers, community organizations, and stakeholders in addressing these systemic disparities.

The report also reveals significant developments in the health equity space. Since 2020, more than 170 health equity bills and resolutions have been passed, with 37 of them enacted since January 2023. It also finds that nearly half of states have 1115 pilot projects targeting health equity goals, with several states offering funding for programs addressing social determinants of health. Additionally, most states now require health equity assessments and screenings. The report guides how health plans can drive lasting improvements in health equity, using New York as a case study.
